Material (RMA)
Procedure
(Please note that this procedure supersedes those found in FSI product manuals or other literature)
1.
Contact FSI to obtain a RMA number. This number should be included on all
correspondence concerning the equipment and included with all shipments.
Please provide a full description of the fault, the part numbers and
serial numbers of the equipment involved and full return shipment
information.
2. Ship equipment to
FSI address. If shipping from outside of the US the equipment should be
consigned to FSI c/o Air-Sea Forwarders, 140 Eastern Avenue, Chelsea, MA
02150-Boston (Logan) Airport, Tel: 1-617-884-8982 Fax: 1-617-884-6119.
3.
FSI will evaluate the equipment and advise of faults found and provide a
repair estimate or advise of warranty status. Please be advised that there is a fee for this service.
4.
FSI will complete repair upon receipt of customer Purchase Order or
prepayment (if not under warranty) and return to customer.
Notes:
Customer is responsible for all inbound and return shipping charges for repairs.
FSI
is responsible for return shipment of warranty repairs; FSI will use best
commercial freight for return shipments unless otherwise advised; Customer
will be responsible for additional charges should a faster means of
shipment be required.
