RETURN POLICY

It is necessary to contact FSI and obtain a Returned Material Authorization (RMA) number prior to returning equipment. The RMA number assists FSI in tracking the whereabouts and arrival recognition of returned goods. Please follow the procedure listed below when returning U.S. origin goods to prevent any delays or additional costs.

  1. Two copies of your commercial invoice showing value for customs and  reason for return must accompany all shipments. Please clearly state on invoice "Returned American Goods for repair".  Failure to do so may cause a delay in customs and additional charges.
  1. If the value is in excess of $1,000.00, the following shipper's oath must be sent with the invoices. (This can be typed on the invoice or on a separate letterhead).

"I, _____________________________, declare that the articles herein specified are, the growth, produce, or manufacture of the United States; that they were exported from the United States; from the Port of _______________________________, on or about ___________________________; that they are returned without having been advanced in value or improved in condition by any process of manufacture or any other means, that no drawback, bounty, or allowance has been paid or admitted thereof."

  1. Ship via courier -UPS, FEDEX, DHL.  If this is not possible and shipment must be sent by freight forwarder route to Boston, MA in care of Air-Sea Forwarders.  Shipments received this way will be billed for customs clearance and inbound freight.
  2. If the equipment is property of FSI, please insure for full value.
  3. E-mail or Fax one invoice, packing list, and copy of the airway bill to FSI upon shipment.
  4. Please refer to the FSI issued RMA number on all letters, documents, and correspondence.
  5. Freight must be prepaid on all returns.
